Custom Dental Abutments
At The Tooth Studio Dental lab most of our abutments are designed in house, and we then send our digital information to these premier companies (listed Below), we have chosen these for their attention to detail and higher standards in their precision in milling, and the quality of their materials.
The main concern in designing custom abutments is keeping costs down and managing the gold cost, or eliminating it all together. There are no shipping costs, and no cost of metal when casting to abutments.
We design the custom abutments as an ideal to tooth preparation, and we leave 2mm of occlusal clearance, and 1.5mm of circumferential when the implants are deep enough. This reduces the high noble metal cost for Porcelain Fused to Metal restorations.
An Ideal design would be a titanium or chromium cobalt coping on a titanium abutment, and E.max™ or Promax™ on zirconia abutments.
The best way to save money is to design a custom titanium, or zirconia screw retained abutment, as there is no metal costs and the restorations are retrieval, and there is no cementing involved.
We design these abutments to achieve many restorative goals. And the emergence profile of zirconia and titanium serves five main purposes.
1.To move the crowns emergence close to the natural tooth to achieve the best interproximal contact. This design will insure us to regain the interdental papilla (implant crown interproximal contact measured to the crestal bone of adjacent teeth is less than 5mm the papilla will be restored 95% of the time) to ensure papilla comes back [Tarnou].
2.We set the ideal margins to help create crown emergence. With the margin heights, ideal cement retrieval is optimized. Eighty percent of all implants fail due to residual cement left in the sulcus. It irritates the tissue and holds bacteria that both agitates the tissue and produces recession, which then causes the bone to resorbs, and finally causing implant to fail.
3. With the emergence profile we design the ideal circumferential thickness to achieve the correct crown emergence and give the proper esthetics, form and function to the tooth. Ensuring that we have no overhangs and concavities, keeping hygiene at the highest level, and prolonging the life of the implant.
4. Ideal gingival sculpting. The abutment emergence profile dictates the neck width and gingival zenith and height.
5. The other reason for custom abutments is to correct the angulation of misaligned implants, and most importantly to change the angle of disclusion in the final restoration, for every 10 degree change in the angle of disclusion, there is a 35 degree change in force applied (DR.L. Wienberg). So we tilt it back to get ideal disclusion, even though the angle is determined by the bite of the patient, we can reduce the stress on the implant by rotating it into a more ideal occlusion, and is especially effective on anterior teeth, so we are able to control the angle of emergence.
The result of turning all abutments into custom abutments is that the restorations are as ideally placed as possible; the cement gap is properly placed for ideal removal, the papilla and gingival can be properly contoured, and the best form, fit, function, and esthetics can be achieved.
The custom abutments that we do not design here at the tooth studio dental laboratory, we send to Astra tech dental (Atlantis).

The UCLA Abutment is a castable abutment. It is offered with a machined gold alloy base or in a fully castable version. It may be used for single or multi-unit screw or cement-retained restorations. It may correct angles up to 30 degrees when cast as a custom abutment. But it is very costly due to casting a high noble metal.

The digital workflow of Straumann® CARES® CADCAM gives modern, quality and business-oriented dental labs the efficiency they need. The initial scan provides the correct data for the CAD design, which is central in your work. The next step requires nothing more than sending it to the Straumann production center. This workflow is highly efficient regarding quality as well as cost and time.
The new Straumann® CARES® Visual 6.0 software excels with a broad range of functions and its ease of use.
The new software allows us to design abutments more easily and precisely.
We can make custom abutments can be made for all straumann implant interfaces. We have all of the posterior necked platforms and all of the new bone level implant abutments in titanium and zirconia.
Strauman® CARES Titanium
Straumann CARES titanium abutment copings allow the practitioner to use CAD/CAM-based customized abutments in any clinical situation. The main advantages of customized CARES abutments are as follows:

Straumann CARES titanium abutment copings are available on all the prosthetic platforms in the Straumann® Dental Implant System: Narrow Neck, Regular Neck and Wide Neck (NN = dia. 3.5 mm, RN = dia. 4.8 mm und WN = dia. 6.5 mm).
The titanium abutment coping is a one-part unit, which means that no StraumannsynOcta® 1.5 abutment is required. The Straumann CARES titanium abutment coping is screwed directly into the implant and torqued to 35 Ncm.

Biomet 3i
The Tooth Studio now has 3i Incise designing software. 3i custom abutments are coming soon. Right now we have the capability to design copings and bridges.

3i Encode
The beauty of 3i Encode healing cap impression system is that you don’t have to worry about impression copings. A normal crown and bridge impression of the special healing cap tells all the details to the encode system. We mount models on a calibrated ivoclar 100 articulator and we mail them to there facility, which places the analogs and designs the titanium or zirconia abutments. So we receive the models back that have both parts ready to restore. We then decide on the type of crown,then we get the coping or substructure made andthen finish implant crown.
In recent years dental CAD/CAM technologies have grown by leaps and bounds, and the Encode® Restorative System from 3i demonstrates the pinnacle of this rapidly growing field. The system begins during surgery with the placement of an Encode Healing Abutment that has has a specific code embedded in the occlusal surface which is only readable by the CAM software. These codes hold specific information necessary for the ideal anatomical design of the final Encode abutment. The reading of these codes allows for a precision made milled titanium abutment to be crafted from a solid piece of alloy without the need for an implant impression, which greatly reduces the time and stress on a patient while allowing for even quicker results.
Impressions of the healing abutment can be taken with any impression material, and neither a cord nor a fixture impression coping is necessary. A counter and bite is taken and then delivered to the lab, which then pours the impression and pindexes the models, and then mounts them in the standard fashion. The pindexed die of the healing abutment is then scanned and from this the final abutment is milled. Unfortunately with present technology the milling machine cannot activate an analog into the model, although soon this feature will be available. Future developments will also soon allow the final crown to be fashioned simultaneously with the abutment.
